PLAYER TRYOUT (EVALUATION) PERMISSION FORM also known as PERMISSION TO SKATE FORM

This process is only for players from the Hamilton ALLIANCE residential Hockey Home Centre who are trying out (attending Player Evaluations) for the Hamilton Steel.

1. Download the 2024-25_Alliance_PTS_HH_Fillable.pdf
Caution: Do not fill out the form when it opens in the preview panel, it must be downloaded
2. Complete the player information
3. "Save As" by adding your Player Name to the file name
4. Attach completed and saved file to an email and send to Hamilton Huskies Registrar Kathleen Sharkey [email protected] for her to sign off on and return.
