    Various forms that a team will require throughout the season to assist them with team administration ----- ADOBE forms that can be filled in may not work properly in some browsers particularly GOOGLE CHROME ----- The "EMAIL FORM" button will only work with computer default programs such as OUTLOOK, not with internet based email such as Gmail, Hotmail etc. ----- If you have problems with the ADOBE forms you should save them first to your desktop or a folder of your choice. Then complete the form and "Save As" with your team name in the file name, attach it to an email as you would normally do and submit to the appropriate person(s).

    2023-24 Team Personnel Information Form
    This form must be completed by each staff member of all Hamilton Huskies teams to provide consistent contact information. This information is important to the Registrar when completing team rosters for approval in the Hockey Canada Registry, it gives the Webmaster permission to publicly post the information you choose and it provides a list for the convenor responsible for Police Checks. THIS INFORMATION IS ONLY USED WITHIN THE HAMILTON HUSKIES ORGANIZATION WITH THE EXCEPTION OF SPECIFIC CONTACT INFORMATION THAT YOU APPROVE FOR DISPLAY ON THE HAMILTON HUSKIES WEB SITE

    This form is to be used when RESIDENCY changes within your Alliance Home Centre or from one Alliance Home Centre to another Alliance Home Centre.

    Residential Move Forms & Process- OHF Member to OHF Member
    This process must be followed when RESIDENCY changes from one OHF Member Partner to another OHF Member Partner ie: from ALLIANCE to GTHL. These transfers are initiated by the individual, not your former or future minor association, and must be processed and approved by the OHF.
